Our Care Plan
Our care plan is based on individual need assessment, actions, and responsibilities. Our objective is to manage our client’s needs, consider all risks, and have contingency plans to meet all actual events.
Delivery Of Services
All services offered will be on needs bases only, and will be performed by skilled and trained professionals. Family members can rest assured that caregivers and health professionals know what to do on a daily basis and also in the event of a crisis.
Elderly Care Planning
Our Elderly Care program aims to shed light on the elderly lives, bring awareness to their situation, and give them a better life and future. It also seeks to improve their living, social, economic, and healthy conditions by supporting them to ensure that they live in warmth, and safety, with some level of independence.
An interview is required for all clients so we can develop a care plan for each client based on the requirements established. The interview will cover areas such as medical history, family history, ADL (activities of daily living) finances, medical, death expenses, and the appointment of a legal guardian is this is required. Long-term care is provided in different places by different caregivers, depending on a person’s needs. Most long-term care is provided at home by unpaid family members, friends and can be very burdensome if the person is not trained especially when it comes to administering medications. It can also be given in a facility or privately within your home, nursing home, or community, for example, in an adult day care center.
The most common type of long-term care is personal care—help with everyday activities, also called “activities of daily living.” These activities include bathing, dressing, grooming, using the toilet, eating, and moving around—for example, getting out of bed and into a chair.
People often need long-term care when they have a serious, ongoing health condition or disability. The need for long-term care can arise suddenly, such as after a heart attack or stroke. However, most often, it develops gradually, as people get older and frailer or as an illness or disability gets worse.
